A structured, personalised discussion
The review considers your current treatment, relevant results and the wider factors affecting your health. Recommendations are based on the information available and should be coordinated with your registered NHS GP and diabetes team.
Important scope limitation
This service is specifically for Type 2 diabetes. It does not provide specialist management for Type 1 diabetes, pregnancy-related diabetes, diabetic emergencies or acute complications.
When to seek urgent help
Seek urgent medical advice if you are acutely unwell, have very high or low glucose readings with symptoms, vomiting, dehydration, confusion or breathing difficulty. Use NHS 111 or emergency services as appropriate.
